International Student Response Form

The International Students Response Form must be completed by the applicant and sent with the copies of Certificate of Education and Passport.

The Form must be completed as accurately as possible presenting correct information. Completing all relevant sections of the Form is advisable.

The Form is to be completed in block letters in Russian or in English. You may send the Form by soft (electronic) as well as by hard copy.
